Oh, well charting by exception means you only chart significant findings in your notes, and leave anything that is within expected/defined parameters out. Basically it just saves time and space, and helps everyone else reading the notes get to the meat of the issue. Sometimes key info can be buried in a note if you include every single little detail.
edit: I know we're not in the same field, but if you need help/pointers with your soap notes, I'm pro at 'em!

Notes are real important for me in the future because that is how I will get paid by medicaid/care and insurance companies. My facility doesn't bill for services but the OT there wants us to get used to it. So I should get some good practice *eek*

Writing soap notes/doing any kind of charting does seem daunting at first. I remember always feeling like I wasn't writing the right thing or the language wasn't right - because at the end of the day, those are considered legal documents. But as time goes on and you become more confident with your assessments, you'll become more confident with your notes as well.
It's kind of a lot of pressure when you're just just starting out, haha.
