I can't really remember how it came about other than that it was the result of a bunch of gay guys, a 24th floor balcony with no railing and some hallucinogens.
Moral Hazard is an insurance term derscribing a person who's actions, behaviour, attitudes or character makes them a greater risk as they are willing to engage in activities that pose a large potential for loss or damage. This became a nickname for me during a class on liability insurance... the instructor asked if anyone could give him an example of a moral hazard; two of my co-workers -who were in the class with me - answered (in unison) "Craig."

Lisa in itself is a nickname b/c that's not my legal name. My real name is Elizabeth - I was named after the Queen of England 
My father is English and when I was born it was the last time Queen Elizabeth was in Windsor. My parents have called me Lisa since I was a child though. (If you don't get the nickname...Elizabeth -> Liza -> Lisa. I can't tell you how many times I've been asked how you get Lisa out of Elizabeth)

teacher was doing a roll call in grade 5, and instead of 'chris park' she called out 'chris pork'
so my friends started calling me pork.

Don't even remember my name came along, but it had to do when me and my buddy had a CD burning business. This was back in the day when no one had one, but me and my friend did, and we could charge $10.00CDN for a burned CD of music, and more for Apps & Games. Oh the good old IRC days.
Anyway, one of my tag names was |PTP| and e|X|tC. I honestly can't remember where those came from or how they developped, but each of our CDs that we made had those tags on it.
Anyway, I guess I have a few nicknames:
- Hack
- Hacker
- Hackett
- Hack Attack
(all derived from my last name)
- Jub Jub
- Jubby
- Jubster
(apparently I reminded this one girl of the iguana that Patty & Selma had from the Simpson...???...and that name stuck for a bit)

My nickname on here comes from something my mom used to say...
she pointed out some girl on tv one day and said "shes got moxy" and I was like "huh? whats moxy?"
and she explained its like... class, pizazz, spunk, confidence, etc. all rolled into one.
So that kinda stuck in my head and I used it as a nickname.
But IRL my name is Sharyn and thus, nicknames are;
Shay (what all my friends call me)
Sugah SheyShey (haha ..given by my ex, dont ask)
Moosh (pet name from ex)
ShareBear (some friends call me this, but ICK)

I don't think anyone under the age of 30 has called me anything but Oreo in about 5 years, hell even some of my teachers called me Oreo. Actually some people do call me Cookie on occasion but that's fairly rare, though I do like the name.
It started in Gr. 3. We were sitting around choosing nicknames for people, well my real name is Orest and somehow the connection Orest - Oreo got made and it's stuck ever since.
p.s. I hate Oreos

going back to nicknames, since I've only mentioned my TA one, most of my ruski friends call me Misska, which doesn't make much sense in English hehe but that's something that has been stuck to me since I was 15-16
my best friend and I call each other hedgehogs (ezhik), I don't even remember where that came from
some of the TA's I speak to on regular basis just call me Funky
