Berlin party

Hi guys,
I am ready to book my tickets for "The Love From Above" but was wondering if anyone could help me with some translation..

I am aware of some of the obvious words such as Kartennummer is Cardnumber and Strasse = Street and Vorname/Nachname etc but need assistance on the rest..

leave them blank (company and notices).

notices you can fill, if you have something to tell to hekticket.

hmm, don't know the form, but this is a case for "notices" i think...
