A remix of a remix? For example, if I were to remix a song by tiesto, which was also remixed by cosmic gate, and in my version I included elements of the cosmic gate version, would I call it Tiesto - Track name (cosmic gate VS mynamehere remix)?

I'm actually not sure about this one either. I think you got it right though.

if Tiesto asked me to do a remix, which also contains elements of the Cosmic Gate remix, I still would call it a "subtle remix" cause it was I who RE mixed the track..

yeah it really depends...if you are really remixing it you could just put your own name...
for example...
Art of Trance - Madagascar (Cygnus X Mix)
is the mix that made the tune what it is...and all the other remixes were takes on this remix, even
Art of Trance - Madagascar (Ferry Corsten Mix)
uses the changes cygnus x made to it in his remix...but it is actually a remix, he didn't just change around the order and stuff...he made his own synth and changed the song...
but if all you want to do is take an existing remix and then take some kind of credit for changing a bassline like paul van dyk...then that would be an crappy attempt at an edit...

Not at all, which is why I'm including cosmic gate in the title to begin with. It's just something I'm doing for fun because I was bored, not like a serious track, and I just happen to be obsessive about naming tracks properly. Basicly I took the cosmic gate melody and rebuilt the track from that, it doesn't really add anything too original to it, it just sounds like a different version of the same track.
I just called it DJ Tiesto - Urban Train (Cosmic Gate vs Xelphore remix)

I'd Go with:
Tiesto - Track name (cosmic gate VS mynamehere Bootleg Remix)
