Painting elephants are a common tourist attraction in thailand, and they're very real. I haven't been, but my GF was just there, and she said that they made "paintings" like christmas trees and stuff out of dots. She didn't see anything like this one, but it looked pretty legit in the video, and I wouldn't discount it as completely impossible/fake.
I can't imagine what it took to train that elephant though.
