They are based on the amount of total votes each received in the first round. The groups are organized based on their ranks derived from those numbers.

1 & 2. I'm disappointed Probspot - Foreplay didn't make it into the Sweet Sixteen. That's quite alright though, it's my 2004 TOTY regardless.
