Yes, you need a receiver or amp. Emu states in their promotional material that they assume most users have one. Aren't your speakers/monitors connected to some sort of receiver or amp? The receiver should have a headphone jack, and that's where you plug your headphones into.
The 0404 has two unbalanced LINE OUTs (it says this right on the box), which are only going to work when connected to the LINE IN of something else. It's preferable to use S/PDIF or ADAT, but if you have a cheap amp with a stereo line in, any decent stereo Y adapter like this one will do the trick. Plug the mono connectors of the adapter into the line outs of the 0404, and the stereo plug into your amp.

This has nothing to do with the card, it's a setting in Windows. Open up the Sounds and Audio Devices control panel, go to the Audio tab, and change the primary devices to the Emu ones. You don't need to disable your other cards - I still have all of mine enabled.
And to all the Emu bashers:
I've had my share of roadblocks with my Emu card, but you have to consider that these kinds of cards are bleeding-edge technology. Up until a couple of years ago, no one had even THOUGHT about attempting to combine software and hardware components. Despite the annoyances, I'd say it's pretty reasonable to expect a few speed bumps here and there while the technologies (and drivers) mature on the market.

The difference between "pro" and "consumer" products is precisely that consumer products are designed to work out of the box, for the average Joe. I'm not really sure if I'd call the 0404 a "pro" card, but if you decide to buy more high-end equipment in the future, expect to have to fiddle with it just as much, because the manufacturers of professional equipment expect that their buyers have the patience and know-how.
