In the second field your username.

In the fourth field put a Password (just choose one, you will only need it once to log in)
In the fifth field, repeat that password.

Now you have to go to your email (which you have put in the third field). You recieved an email from:

Go back to www.dancetour.nl and log in (left at the top, under 'MEMBER LOGIN')
Now you are logged in and you can vote for Set 2 - Jay G (in the middel on the leftmenu)
Click on the button 'Stem!' and your vote will be counted!!
