Probably because there is no error.

The thing that is wrong is that someone with a bad stuttering problem would recite each letter an extra 2 times

I'm guessing the 20% that found errors are idiots.

This is the stupidest nonsense. The OOO in the actual question are supposed to be 000, and supposedly most of the people given the test couldnt make out the difference between an O and a zero.
