Your quote makes a good point about people exploiting the isolation and turning it higher, I often do that, but still nowhere near as high as what I hear in a nightclub.
I think if your ears are ringing for days it's fair to say you've done more damage to your eardrums. If I have my earphones ridiculously loud the transition feels a bit weird for a few seconds after taking them out, but that's it. A night out in a club can leave your head - and espescially ears - banging throughout the next couple of days. When I come back from a night out and the house is silent the silence is deafening that I can't get to sleep without music.

I agree that if you use common sense you won't hurt your ears while using headphones. That's why I like my Sennheiser HD 280 Pro's, you don't need to blast them to get quality sound and they've never caused any ringing.
