FreeRecordShop has it for €14.28 (Euros). Note the price will say €16.99, but there is a €2.71 VAT deduction as you proceed through checkout as it's tax free for non-EU countries.

Shipping is probably €3.00 (they aren't too good at letting you know, but it's always been €3.00 for me.)

In the end, it should be right around $20.00 US to get it no matter where you live in the US, and that includes the CD and shipping.

I use FreeRecordShop whenever I need something that's really only available in the Netherland (usually event DVDs ) and have never had one problem with them.

It actually varies. Anywhere from 1-3 weeks. Sorry for not making that clear.
