I have two friends that wanna have them reduced bc they are insecure and overly self-concious because their breasts are so large. They do not have back pain problems and prolly will never develope them, but are merely WAY to self-concious w/ the size of their breasts (being that they're too big).
I'd still have a hard time telling them it wouldnt be right therefore if they had them reduced to make them feel more comfortable w/ themselves day in day out.
And then if its "ok" to reduce them for the sake of making urself feel more comfortable - I feel there shouldn't be a problem w/ the reverse, enlarging them to make urself more comfortable. I dont even enjoy implants, but if one feels that will help, well, its ONE of the ways out there to improve your self esteem.
I agree there are other ways too, so one shouldnt rule them, but I dont feel there's anything wrong w/ taking the enlargement (or reduction) route... just be aware its a more drastic method (and one w/ possible side effects).. but all that is just something you have to weigh into the equation when you do it.

EDIT: That's just my take - I did see where you posted that you dont support ANY kind of plastic surgery save for in 3 situations you listed above.
