Over here, it's the standard in most better places to do it medium-rare. I don't know why, but it is. Medium usually gets you a slightly pink, but cooked piece of meat. Medium-well gets a fully-cooked, grey one. I've never had to send back anything I've asked to have medium.

I usually don't worry about spit in my food at restaurants...now fast food thats a different story. I've heard too many disgusting stories from people who have worked at them. One person said he served a hamburger with his ball sweat on the bun. When asked "why on the bun and not the burger?" I was told, "the bun is more absorbent to insure he will eat as much of my ball sweat as possible." Yeah, don't piss someone off who works at a fast food place and then order food there.
