I've not left the country since owning any major credit cards, but I *did* have an active passport that I used to travel about Europe from 1996-2003. I don't think I used any cards overseas though. I still received this form though and opted for the easy payment of $25... just cuz' it's liek...free monies.

From my understanding of how it was worded, I was entitled to the 'easy' option due to merely travelling in and out of the country within the indicated time period.
