Actually, I don't think I've ever been annoyed by that. I've had a dining experience ruined, however, not by a waiter, but by the person I invited.
I once went to an Outback Steakhouse for my birthday some years ago, and the place was packed. "Well, it IS January", I thought, "we're all on vacation, and in a shopping mall: let's ask them to add us to their waiting list, and find something else to do while we wait". And so we did.
We played mini-golf, saw some bookshops and, after an hour or so, they beeped us. Everything was fine... Or so I thought: One of my friends suddenly started throwing fits out of the blue because she thought they were slow and she was hungry. All I wanted was a bloomin' onion, and when I ordered it, they told us they weren't serving onions that night. I didn't really care, and I soon looked for something else to eat. It was crowded anyway, and I just wanted to have a snack.
But, this friend of mine was incredibly enraged, and she kept arguing with the waitress, who couldn't really do anything about it. My friend kept yelling hysterically at the waitress saying that she wanted to see the manager until they phoned the guy and he apologised to her because they had run out of onions... and that's when I eventually lost my appetite and decided to have just some coca-cola =/
