I think it goes along with all other things, its not really a choice.

One of my good friends is gay, and hates being gay. He has a slight lisp, and acts a bit feminine, but tries not to. Every now and then, it just slips out.

Although, I am sure you are right that some of them do accentuate it, and just run with it.
