I dont do it either way

i make up a stupid, embarrasing nickname.......

so when that person calls and someone is like "who is FAGBOY?"

i can say, "oh that's your brother man...yeah...erm...sorry"

Most of the time its just their first name.

If I know two people from two different places I write where i know them from, eg Dave Uni and Dave Swim (swim = my old swimming training buddies)
For TA's i put first name and screenname, eg Ben Nutter
For people from work I put work on the end, eg Jane work (or for the department number i flip it around, eg Work Ashford, Work Head Office)

That usually suffices.
