I fell down the stairs going from the second floor down to the first floor once.
Not the whole way, only some of the way. just enough to bump my head.

i have a fear of falling down stairs at Vision -- the spiral staircase, and the stairs just behind that that go from the 2nd floor to the dancefloor (there's no railing on the wall side).
