On a fundamental level this is how it works. Your computer, or computer system, is full of access points (doors). For example, a house has a front and back door, a garage door, and windows. A burglar trying to get in will use the easiest way possible but a diligent homeowner has locks and security system. Just like computer/systems have firewalls and security software. A hacker trying to hack a computer/system will first try to gain entry through the front door. When that doesn't work, they try the back door. If that doesn't work, the windows. Computers have far more, so much so, that it is essentially impossible to have a 100% secure computer/system. Even if all the doors are locked, I can trick you into opening the front door. Or when you decide to leave your house, I can sneak in while the door is open. There are so many ways to hack/burglarize a house/computer/system.

Thats a pretty good description, except that most of the time its more like "they forgot to build a wall there and it also happened to be the wall that lets you into the locked vault"
Its deadly easy if you know how to find exploits in systems. Its not even malicious knowledge half the time. When you work in computers, especially in web development you know how to do it cause you are trying to make it so no one else can do it.
