I've seen some awful dj's in my day, but hands down the worst performance by a top name dj for me goes to two people...

Paul Oakenfold (Numerous Times)
I'm not sure why I see this tool play live anymore (maybe free tix help, I don't know), but I haven't seen him play a decent set since 2000. It's as if he simply doesn't give a damn anymore. He's too busy pumping his fists into the air with a smug satisfaction that he's the king of "playing other people's music" to acknowledge the skill he supposedly has. I mean why bother, right? He's Paul Oakenfold, an innovoator of the modern rave. Mr. Shoom and whatnot? Mister Ibiza Baleric Trance, right? Let's forget that he pioneered progressive trance (by whoring out Goa Trance). Let's not forget that his "big break" was opening for superstar band U2. Yes, he's accomplished and his contribution to the music is great. But then again, so is his ego these days. So much for being humble.

BT (Live Performance - Electric Daisy Carnival 2000)
Okay, I've got to give the guy props for pioneering new technology (one of the first to take on Ableton Live) and the guy really knows music inside and out. But this performance was the start of a series of sets in which I've come to notice that he indeed is too smart for his own good. He's a bit too eager to take on technology and it shows in his performances. Worst part is that at the end of his performance, he thrashed the gear onstage in a psuedo-rockstar fashion. Oh yah, the music was still playing despite just about every piece of gear being toppled over.... Curious-er and curious-er, I say.

I think the question is not specific enough. For example, if a DJ played many great tracks but couldn't mix for shit, IMO it's not memorable but the t/l makes up for it. If he was just crap all over the place.. tunes, mixing, everything then yes, that's what should answer this question.
