From my understanding of seeing various vinyl / cd and digital releases thru the years.

Original Mix - The absolute original 'track' that the artist mixed down (the most common of the lot)
Extended Mix - An extension of the original mix, adding some bars / drum loops to make it a bit longer.
12" Mix - Quite possibly, the Original mix, but edited so as to fit the time frame of a vinyl disc (assuming, the original mix is longer than the limit)
Instrumental Mix - Absolutely no vocals whatsoever, be it the chorus or any of the verses (There's usually a vocal mix present in this case.)

Dub Mix - Usually, just the chorus, not any of the verses.
Club Mix - As mentioned earlier, dance floor-oriented version of the original,more drum-loops, longer intros, basically dj-friendly.
There can be variations of the above, Original Club Mix, Vocal Club Mix , so on and so forth, as the artist chooses.
