I had the bomb chest/back workout today; One of the best pumps I've ever had.
Incline DB: 12x85, 10x95, 8x105, 6x110 dropset to 6x55
Chins: 4 sets, 10 reps (and no kickin my legs up either! )
Dips: 4 sets, 12 reps with a 45 DB 'tween my knees
Bent Rows: 4 sets, 12x135
Incline Cable Cross Over: 12x35, 12x40, 12x45, 10x50
Seated Rows: 12x150, 12x170, 12x190, 10x210 dropset to 6x110
Close-Grip Bench: 15x135, 15x135, 10x185, 10x185
Bent DB Rows: 12x25, 12x35, 12x35
I'm blitzed right now but I'm going back again tonight to play basketball. Hopefully I can lift my arms to shoot!

I'll make a suggestion to you, a pretty simple one but surprisingly effective:
Think up one or two goals, write them down, and start from there. Maybe your first goal can be to work out 2 times this week. Simple enough! And then build off of that and soon you will see it snowball. But KEEP WRITING THEM DOWN until it is just second nature for you to do these things. Here is a story you might be able to relate to:
Each week I set aside a certain amount of money for vinyl. And then I go to the record shop or online and spend that money. Well it first started out that I would go and just browse and see what they had and make my purchases that way. Alot of fun yes but, then I looked at my vinyl collection and saw just a bunch of random shit! So now I have written down a master list of the vinyls that I want; 3 weeks out of the month I have to buy from that list, 1 week out of the month I can look for random stuff. Now my collection is growing in a more desirable fashion - as long as I stick to my plan.
My point is, you'll be more likely to get your results and stay motivated if you know what you're working towards and you've set expectations for yourself. Hope this helps!
Edit: Make sure you include dates in your goals; they should always correspond with a timeframe, that way you force accountability on yourself. And one more thing, if I ever don't feel like working out I think this to myself: "I bet Richard Simmons is working out right now, no way am I going to let that bastard outwork me!"
