I think better examples could be used than this, since chernobyl was mostly due to human error. |

| u could research the event that happened with an airbus crashed in syberia...the 15 year old kid of the pilot accidentally discovered a huge flaw in the plane's autopilot...he actually made it disable itself... and then he crashed the plane and unfortunately everyone died...the investigators used the cockpit voice tape and a flight simulator and then figured out that the autopilot had that huge defect... |
