I think it's an assault on the patient if they perform such an act strictly against his specific wishes and have to resort to holding him down physically and sedating him with a needle in order to do it.

Patients have the right to know ahead of time why certain examinations are done and what discomforts might be experienced before they give their consent, if they are in a state where they can provide consent, i.e. not unconscious or hallucinating. |

could he not have been "hallucinating" due to the head trauma? which would explain why he got violent in the first place? |

Actually, checking someone's "rectal tone" is a good way to tell if they have spinal nerve damage. They do this all the time to children in the ER.
