For the sake of discussion I was in the loony bin a couple of weeks ago. I feel like it's only a matter of time before I get a label, maybe its this one. |

I encounter people with BPD daily at the psychiatric unit where I work and especially those which are in highly unstable (suicidal) states due to me being on the closed section.
Manuals and internet information which is available on the internet does not show a full picture of the disorder since it varies a lot. I've met people which were extremely isolated, dependent on their relationships, anxious but simultaneously extremely courteous and friendly. On the other side there are the aggressive, dramatic and manipulative types which had also attempted grave suicide tries (throwing oneself infron of a train or having over 70 sexual partners) on numerous occasions and who would start screaming and attacking us doctors when we denied them benzo-drugs.
It is true that while they are extremely manipulative, this can be seen from a long shot due to a sudden shift in their behaviour and, in my experience, has to do with them wanting higher dosage of medication.
BPD is often mixed with depression and the distinction between BPD, bipolar disorder and schizoaffective disorder is often hard to diagnose and requires quite a bit of practice in knowing the patient and how they behave. There are many BPD that fake symptoms such as hallucinations in order to garner attention. For an untrained eye it is hard, but during discussions there are different methods for identifying the degree of truthfulness in the words. I'll give an example for a patient I spoke with a few days ago:
My chief told me to have a conversation about how the weekend permission went. The patient (woman) told me that although it went well she had suicidal idealization and anhedonia and wanted to die as well as a hallucinations that someone tried to enter the apartment and kill her. I asked her if that made her afraid, to which she replied "yes". "How come? Whether you live or die is irrelevant".
She sprung up and got stuck in her own words and thoughts.
I've met patients with true hallucinations and the above description falls off the bat by a long shot.
It's all about experience in the end. And the medical treatment for BPD isn't a pretty sight either. |

How many of you who suffer from Borderline Personality Disorder also have Type I/II Diabetes Mellitus?
