Contra without the code isn't tough at all, I have done it and I've known several people who have as well.
I'd say, anything involving eagles in Ninja Gaiden (NES) or anything involving Medusa Heads in Castlevania...

Dunno which ones harder, defeating the final boss or beating nemesis in the plane.

Intelligent Qube (a puzzle game) for PS1. Beating the game was simple enough (a 45 minute trek), the hard part was beating it without dying once. Using up a life meant restarting the score to 0(the score would be run through an equation to create an equivalent IQ level for the player). The game looked like complete with a simple control scheme, but was amazingly ADDICTIVE.

Oh, I remember that game, even if I only played the demo. It was called 'Kurushi' or something like that in the UK.

Final Fantasy 1 when you would setup an attack and then the target would die and you end up losing a turn because it didn't automatically default to an attack on another enemy meaning that you could have used that turn to heal but instead your Red Mage gets killed and then you have to limp through the rest of the fight because someone else gets poisoned and you're struggling to stay alive and can't spare a turn to revive your mage and.......
