I'm well aware of the role that diet and exercise play, but focusing on those two things alone won't cure depression.

i never said it would cure depression, but as far as prescribing goes, it should be the first thing on the list, and in all cases it couldn't hurt. you can't really say the same thing about any of the depression meds out there. nevertheless how many physicians even touch on diet and exercise i wonder? the other obvious contributor is drugs/alcohol, but we all know that one's a no go for most.

Also, how many depressed people do you know? Because you just try getting a depressed person to feel interested/motivated enough to change their diet and exercise right away.
It's very easy to say that if you don't understand the illness.

well i mean the same thing can be said about fat people getting lean, criminals going legit, addicts getting clean, morons getting sharp. no one said improving yourself was easy. imo people are completely plastic and what you are today is a result of habituation. if you can will yourself into breaking a habitual cycle for more than six months, for a year, then you can turn any corner you choose. it's just easier and more comfortable not to.
honestly i don't know any depressed people, but that's probably because i don't cross paths with them in the places i go, people i see and things i eat. i'm drawn to healthy, intelligent, self-actualizing people. my roommate in college used to have huge manic-depression swings, but he turned a corner years ago through strength conditioning and healthy diet. maybe that's why i evangelize it. i can attest to the effect of ty eating on mood because i used to feel lethargic and unmotivated every morning i woke up in college and when i stopped eating processed foods daily, that changed almost immediately. and while i'm not immune to stress and poor mood, when i go on a run, a bike ride, a climb or hit the weights, i always find it straightens my bearings.

Personally, when I suffered my major depressive episode I found illegal drugs, casual sex, and reading Nietzche helped me out... maybe doctors should perscribe E, whores, and Beyond Good and Evil.
