122 to 138 in 2 hours it's not very difficult, in fact there are many transition songs that are originally at 125bpm and djs play them up to 135. Dustin Zahn - Stranger to Stability (Len Faki Podium Mix) this for example, if you check the tracklistings of trance djs you'll see the bpm gap they covered with this one.
For example Armin once played this track (125 bpm) and after that W&W - Mainstage (136 Bpm)
Looks good so far, there are real jewels, from relaxing melodic ones to club fillers.

Does he pitch it up slowly throughout the song? Rapid tempo changes don't seem to bode well on the dance floor...unless its used as an effect of sorts.

I suggest it to you to try yourself and see what happens, this kind of songs have pitched sounds all the way, you can increase it fast that the changes on it are minor, i have different tracks that work for this:
This 3 tracks are esentially copies of Stranger to Stability.
There are also songs that are designed to make switches between genre, they last for 2 minutes or less and start at 80bpm for example and end at 128bpm so you are playing rnb/hip hop and when you want you play this track and the change is not very noticeable (at least you keep the flow of the mix)
Check this around the breakdown:
Essentially what it does here is pitching the track down during a white noise section, noise and hats generally hasn't got a pitch so you can even feature this technique on your mix. Filter + Delay/Reverb , you pitch it up/down and then you drop both tunes.

the song is being pitched down while the beat is playing so it makes that part a bit awkward, but when the song accelerates back to a faster tempo it does work as a sort of transitional effect
